I love small games. and specialized systems. They have a passion (in most) that larger games (not all) have lost.
Is there a type of game you love (super hero, spy adventure, etc) that is not in a world setting?
Such as: If there was no Shadowrun, and you had to run D&D in a modern setting to get close to the feel.
Or no WoD and you ran TMNT with vampire bat, and wolf characters to suit the feel.
Alternately, if your world exists, what would you change to make it better?
I will say I love post-apocalyptic settings and games, but most are too mad max and not enough book of eli. The difference to me is how much of a social structure is left. The Postman would make a good setting choice for me, giving some people who want order, and a large group that wants power.
